Health and Safety (Survey)

Run a survey using questions designed in collaboration with Worksafe NZ to assess health and safety at work.

Safer organisations perform better. But safer organisations don’t occur by focusing on hazards and incidents. Safety is a natural consequence of focusing on resilience. Our Health and Safety set is designed to elicit conversations centered around the behaviours that have the most impact on resilience.

We recommend keeping surveys below 10 minutes to complete. Long surveys can cause fatigue and result in poor data quality.

Survey structure

    • Questions are grouped by category to ensure that the survey is easy to follow for participants.
    • We recommend asking 3 - 10 rated scale questions followed by 1 open text question for each category, as survey's are naturally measurement focused over conversational.
    • The open text questions for each category relates directly to the rated scale question asked immediately before it to ensure a logical flow for the participants.
    • Including messages, instructions and encouragement throughout the survey helps with understanding and engagement.

📝Note: This survey can be also be run as a weekly conversation set instead.

Part 1 (Capacity)
